South Africa Asks for Help in Cholera Outbreak; Thousands Are Ill and 59 Are Dead
JOHANNESBURG, Jan. 6 - Struggling to contain an outbreak of cholera that has stricken more than 15,000 people along the country's eastern coast, South Africa enlisted the help of the World Health Organization this week after cases surged over the last few weeks.
Epidemiologists from the organization, expected to arrive this weekend, will help health officials map out a strategy to curb an outbreak of the acute water-borne intestinal disease, which has killed 59 people since it began almost five months ago. More than 450 people are currently hospitalized with the illness. Cholera, which is spread by contaminated water and food, causes intense diarrhea that can swiftly lead to severe dehydration and death if untreated.
"It's a typical South African outbreak, where you have large numbers living in poor circumstances, in the rural areas, and those are the people who are infected," said Dr. Willem Sturm, chief of microbiology for the provincial health department in KwaZulu/Natal, site of the epidemic.
"At the same time South Africa's health system is able to cope with that," Dr. Sturm said. "That's what makes them different from most African countries - they have a first- world health system, and that's what saves them."
The most populous of South Africa's nine provinces, with about 9 million people, KwaZulu/Natal is also one of its least developed. Almost 60 percent of its people live in rural areas that are more densely populated than many similarly undeveloped places elsewhere in the country.
The onset of the summer rains, coupled with the annual holiday season migration to rural homesteads, has worsened the problem. Public concern over the outbreak has increased as well, especially in the last week with the first cases reported outside KwaZulu/Natal.

금주 말쯤 도착할 것으로 보이는 WHO의 유행병 전문가들은 이곳 보건 관계자들이 급성 수인성 장질환인 콜레라의 발병을 억제하기 위한 전략을 마련하는 일을 돕게 되는데, 5개월 전에 발병한 이 질환으로 지금까지 59명이 목숨을 잃었다. 현재 이 질병으로 입원한 사람수는 450여명이다. 병균에 오염된 물과 음식으로 전염되는 콜레라는 심한 설사를 일으켜 치료를 받지 않으면 곧바로 심한 탈수증을 일으켜 숨지게 된다.
콜레라가 번지는 지역인 콰줄루/나탈주의 보건부 미생물학 책임자인 윌렘 스텀 박사는 "콜레라는 농촌지역의 가난한 환경에서 많은 사람들이 살고 있는, 남아공화국에서 발병하는 전형적인 질병으로서 바로 이들이 이 질환에 걸린다"고 말했다.
스텀 박사는 "동시에 남아공화국의 의료 시스템으로 이 질병을 감당할 수 있다"고 말했다. "바로 그런 것이 아프리카 대다수 국가들과 다른 점이다. 남아공화국은 세계 제1의 의료 시스템을 갖추고 있기 때문에 환자들을 구할 수 있다".
남아공화국 9개주 중 인구가 약 900만 명인 콰줄루/나탈주는 인구가 제일 많으면서도 개발상태는 최저수준이다. 주민의 근 60%가 농촌지역에 거주하고 있는데 농촌의 인구밀도 또한, 비슷한 저개발 상태를 보이는 다른 여러 지역보다 높다.
여름철 우기가 시작되는데다, 해마다 이때가 휴가철이어서 많은 사람이 시골 농장으로 떠난 것이 문제를 더욱 악화시켰다. 한편 콜레라 발병에 대한 일반의 관심은 특히 지난주 콰줄루/나탈지역 바깥에서 첫 환자가 보고되면서 크게 높아졌다.
